We write further to your request which was received on 24th December 2018 and states:

Question 1: "When were the recommendations in the Mersey Internal Audit Agency (MIAA) activated? Question 2 Will the MIAA be asked to carry out a further review so the public can be assured that all the failed areas have been rectified?"

Our response to your request is as follows: I have detailed below the information that is being released to you.

Question 1: When were the recommendations in the Mersey Internal Audit Agency (MIAA) activated? The hospital started making significant changes to the way that the endoscopy service is managed immediately after the issue (1st April 2018) around the provision of endoscopy appointments for patients who were awaiting a follow-up was noted. This included increasing capacity to ensure there is sufficient appointments for those requiring regular follow up, changing the methodology for tracking these patients through our computer system, and also tightening up our policies and processes.
Shortly thereafter, we strengthened our clinical leadership arrangements and have commenced on the programme to get the unit Joint Accreditation Group (JAG) accredited, which is the national standard for endoscopy units across the UK.

Question 2: Will the MIAA be asked to carry out a further review so the public can be assured that all the failed areas have been rectified? As we are regularly monitoring progress against the MIAA Action Plan at the monthly Endoscopy Governance Group meetings, we are not planning to ask MIAA back to undertake a re-audit, although this has not been completely discounted at this stage.
