Our response to your request is as follows: I have detailed below the information that is being released to you.

The tables on the following pages show for each period, the Provider & Dental Surgery, the Units of Dental Activity (UDAs) which were contracted to the Provider in the given year and the number of UDAs completed alongside the % of completion.
We have also included information, if a Provider had not achieved 100% of the contracted UDAs in the given year, on what agreement was made between the Department and the Provider regarding the reimbursement of any shortfall.
