Hi, It is unclear which board/sub-committee/dept/etc reviewed yellow card data for the Isle of Man. Please provide: 1. All correspondence between DHSC, DfE, Manxcare and Cabinet Office (inc Public Health) related to MHRA, Pharmacovigilance, Yellow Card, incidents and adverse reactions to the Covid vaccination sent and received during the months of December 2020 and December 2021. 2. All information used (including correspondence) to prepare Minister Ashford's response to the Tynwald Question 3.3. COVID vaccination - Issues reported through the yellow card system which was answered in the Tynwald sitting of 15 April 2021. 3. Kathryn Magson confirmed via email on 25 November 2021, that FOI 21/401 'has gone through the vaccination governance processes'. I have not seen this FOI discussed in minutes of the vaccine board under DHCS/Manxcare. - Please clarify what the 'vaccination governance processes' are and provide documents relating to that process? - If not already answered, please state which committee/sub-committee/dept/board was responsible for discussing and reviewing yellow card information. Please provide minutes of those groups. On the 23 October 2023 we wrote to ask you for clarification of your request: In order to identify and locate the information that you have asked for we need some further information from you. In particular, it would be helpful to know where you have stated in the first part of your request: "1. Hi
I am trying to see if the coroner has signed the death certification yet, but ultimately at the moment suggest we should be saying –
We are aware of an individual who sadly passed away last week
They had had their vaccination earlier that day (which we have to say now I think, but
don’t think we should be talking about this more broadly)
The cause of death is not known at this stage as this may sadly have been due to
underlying conditions.
The DHSC are assured that the relevant yellow card protocols have been followed in
relation to the vaccine.
Continue to state that the risks of not having a vaccine etc…….
That’s all I would say for now? Just my thoughts on this for now. Meanwhile will check in around the coroner position.

Actually I didn’t say they died after an adverse reaction to the vaccine. I stated clearly we should not jump to conclusions that the two events are linked and that there currently was no proven link as will be shown when the Hansard is published.

take it down but will need to give a statement putting the record straight:
The Health Minister has confirmed a person died after an adverse reaction to a
COVID-19 vaccine.
David Ashford confirmed the death in this morning's sitting of the House of
Keys.
They suffered anaphylaxis and subsequently passed away.
Mr Ashford says it’s important to consider it could have been as a result of
another underlying health condition rather than purely being administered with
a vaccine:

Hi All
There has been a mix up here.
There hasn’t been a death in relation to anaphylaxis – that was a separate issue - a yellow card, and this was followed through. All protocols worked. Patient discharged.
The death was following Pfizer, cardiac arrest at home – but we don’t have the outcome of the post mortem to know if this is linked at all. In fact it is highly likely that it might not be linked. We really shouldn’t be talking about this individual as we don’t have their consent and we don’t know all the information about this case as yet. The coroner will need to decide – haven’t been advised that this was the case but can ask.
I am not sure where Daphne Caine is picking up the issues around a death re anaphylaxis.

Office does not hold some of the information that you have requested.
Our response to your request in respect of the information that Cabinet Office does
hold is as follows:

The Cabinet Office does not hold information in this regard, however you may wish to submit a freedom of information request in this regard to DHSC and/or Manx Care.
While our aim is to provide information whenever possible, in this instance, some of the information contained in the supplied records are exempt from disclosure to you. Redaction has been applied where the following exemption, allowed under the act, has been assessed and engaged:
